Terms of Service

Formlark is an AI-assisted survey builder. You may use it to create, share, and analyse surveys, subject to these terms.

You must be able to form a binding contract to use the service, and you are responsible for activity under your account.

You are responsible for keeping your login credentials secure and for all activity that happens under your account.

Provide accurate information when you sign up, and keep it up to date.

Every new account starts with a 30-day Premium trial that requires no payment method.

When the trial ends, your account continues on the limited Free plan unless you subscribe to Premium.

Paid plans renew automatically until cancelled. You can change or cancel your plan at any time; downgrading keeps your surveys and re-applies Free plan limits and branding.

Do not use Formlark to send spam, collect data unlawfully, infringe others’ rights, distribute malware, or interfere with the service.

You are responsible for the content of your surveys and for obtaining any consent required to collect responses.

You retain ownership of the surveys you create and the responses you collect. You grant us the permissions needed to host and process them so we can provide the service.

You are responsible for complying with privacy and data-protection laws that apply to the data you collect from respondents.

We work to keep the service available, but it is provided on an as-is basis and may change or experience downtime.

We may add, change, or remove features over time.

To the fullest extent permitted by law, Formlark is provided without warranties of any kind.

We are not liable for indirect or consequential damages, and our total liability is limited to the amount you paid for the service in the twelve months before the claim.

You may stop using the service and close your account at any time.

We may suspend or terminate access if these terms are breached or if required to protect the service or other users.
